Warranty
All ProbeSource transducers are warranted against manufacturing defects from the date of shipment from our facility. Warranty is voided by any misuse or abuse of the product. OB/GYN, Vascular and Cardiac Probes are warranteed for 12 months. TEE and Endocavity Probes are warranteed for 6 months. If a ProbeSource transducer is deemed defective while under warranty, NAI, at its option, will either repair or replace the defective probe.
If a defect should occur in a ProbeSource transducer while it is still under warranty, it should be returned to NAI immediately. Please call customer service at 800-288-8823 to secure a Return Authorization Number. The RA number, along with the ProbeSource serial number, must appear on all accompanying paperwork. In addition, please provide a brief description of the defect to expedite evaluation of the returned probe.
Exchange
All probes are sold on an exchange basis. Your replacement probe will be shipped to your facility in our unique ProbeSource carton. Place the exchange probe in the form-fitted, freight-prepaid box for return to ProbeSource. The carton has been pre-coded to insure proper credit; no other authorization is required.
Exchange defective probes must be returned to North American Imaging within 30 days to receive credit for the exchange. Failure to return the exchange unit will result in additional charges.
Important: All ProbeSource replacement probes are pre coded with a serial number. To insure proper credit, this number must not be removed from the probe.
